We ask all parents and carers to complete the form below for each child individually.

It is essential that the school | nursery holds accurate and up-to-date information, including current addresses, telephone numbers and any relevant medical details, allergies or safeguarding concerns. This enables us to respond promptly and appropriately in the event of an accident, emergency, or welfare matter.

If you have more than one child at the school | nursery, a separate form must be submitted for each child.

Completing the forms individually ensures that all pupil records are accurate and supports the school in meeting statutory requirements, safeguarding responsibilities, and the welfare of every child.

Should your circumstances change throughout the academic year e.g. change of address | contact number, you will need to submit a new pupil medical & welfare form, in addition to informing the school office.
